About TransforMED – Who We AreTransforMED strongly believes that a positive future for primary care practices, family medicine practices — and the health of the patients they serve — can be achieved, in part, through transformative practice redesign. In fact, the genesis of TransforMED was inspired by the practice redesign recommendations of the Future of Family Medicine's report that called for "a financially self-sustaining national resource (to be) implemented to provide practices with ongoing support in the transition to the New Model of family medicine." TransforMED's experienced staff is comprised of experts in the competency areas of the model of care. TransforMED also collaborates with outside researchers, consultants and a diverse group of specialty, industry and policy stakeholders. TransforMED's board has a deep and vital understanding of the uniqueness and relevance of both the “very special specialty” of family medicine and the urgent necessity of proactive primary care. The rich knowledge, expertise, skills and commitment of the staff, board and other key players are complemented by an emphasis in the specialty of family medicine. Accordingly, TransforMED is well-positioned to serve as the hub of a community of patient-centered primary care providers beginning with early adopters through the National Demonstration Project and then radiating throughout the industry, always with measurable results. Funding & Financial DisclosureTransforMED is a not-for-profit transformative practice redesign initiative of the American Academy of Family Physicians and has an $8 million start-up budget. The TransforMED team members are paid by TransforMED for their work on this project. The evaluation team of investigators are paid by TransforMED for their time and expenses for work on this project.
