Both TransforMED and the TransforMED Patient-Centered Model have their origins in the recommendations of the Future of Family Medicine (FFM) report, which called for the creation of a national-level organization that would evaluate, support and guide family and primary care practices to adopt a new, integrated model of care.

Inspired, the American Academy of Family Physicians (AAFP) founded TransforMED and then initiated the two-year National Demonstration Project (NDP) — the most comprehensive PCMH evaluation effort to date. TransforMED’s experienced, clinically-grounded leadership and staff comprises subject matter experts in the competency areas of the TransforMED Patient-Centered Model.

TransforMED is an active member of primary care communities and recognizes and supports the unique value that primary care offers to patients and the health care system. TransforMED shares and supports the idealism and altruism of primary care physicians and the strong commitment to their communities and patients through continuing patient relationships and independent decision-making.

TransforMED is a subsidiary of the American Academy of Family Physicians.
