What We Do

TransforMED is at the vanguard of the new Patient-Centered Medical Home movement to effectively and significantly improve health care for Americans while increasing satisfaction for family physicians and their practice team.

TransforMED acts as a leader and catalyst to generate positive transformations in family medicine and primary care, by using and refining a core group of best practices-- TransforMED's Patient-Centered Model. We focus on which specific actions practice teams must undertake to achieve improvement, and how they can get the most useful results from their transformation while minimizing risk, difficulty and uncertainty.

TransforMED provides hands-on Medical Home Facilitation, retreats, and tailored training that help primary practices measurably improve in three critical areas: patient care, financial sustainability, and the well-being and satisfaction of the physician and staff.

TransforMED also fosters ongoing collaborative relationships among all stakeholders, especially key players in family medicine and primary care, by providing collaborative online tools and resources that promote peer-to-peer relationships and learning community.

TransforMED engages with top experts from within the specialty, industry and policy arenas and disseminates its findings through presentations and learning events.

And TransforMED is forging partnerships with vendors, developing customized and integrated product and service packages, and refining existing products to meet the needs of primary care practices seeking to become Patient-Centered Medical Homes.
