What We DoThe Future of Family Medicine has amply demonstrated the need and value of transformative practice redesign. It is the overwhelming consensus that change is necessary— and the time is now. TransforMED is at the vanguard of this new movement to effectively and significantly improve health care for Americans while increasing satisfaction for family physicians and their practice team. TransforMED acts as a leader and catalyst to generate positive transformations in family medicine and primary care, by using and refining a core group of best practices pre-identified by the Future of Family Medicine. These best practices are do-able, practical and can be tested immediately. We focus on which specific actions practice teams must undertake to achieve improvement, and how they can get the most useful results from their transformation while minimizing risk, difficulty and uncertainty. The National Demonstration ProjectThe initial focus of TransforMED's expert staff is to develop, implement and evaluate a "proof-of-concept" National Demonstration Project (NDP) to pilot test the new model of care in 36 family medicine practices of varying sizes and profiles across the country. The goal of this project is to provide an objective view into how implementation of the components of the new model of care impact patient satisfaction, quality of care, and practice efficiency and economics. It also will help determine the most efficient and effective way to promote transformative change in primary care practices. How We Do ItTransforMED will use the evidence and experience gained from the NDP to generate a validated, integrated TransforMED Medical Home Model that measurably improves three critical areas: patient care, the financial sustainability of family medicine and primary care practices, and the well-being and satisfaction of the physician and staff. To accomplish these goals, TransforMED provides hands-on consultation advice and tailored training to the National Demonstration Project practices. TransforMED also fosters ongoing collaborative relationships among all stakeholders, especially key players in family medicine and primary care, by providing collaborative online tools and resources that promote peer-to-peer relationships and a sense of community. TransforMED consults with top experts from within the specialty, industry and policy arenas. And TransforMED is forging partnerships with technology vendors, developing customized and integrated product and service packages, and refining existing products to meet family physicians' needs. The FutureFuture plans call for TransforMED to use its expertise and experience implementing the new model to expand its service offerings to primary care physicians across the country, providing them with fully integrated suites of products and services. |
