Touchstone Group

The Touchstone group is a continuation of the collaboration between TransforMED and many of the practices that participated in the Medical Home pilot National Demonstration Project (NDP).

At the conclusion of the NDP facilitation component, all but a handful of the participating practices, both facilitated and self-directed, expressed an interest in maintaining the momentum and the relationship network to continue their improvements and validation measurements of the medical home. Thus was established the Touchstone Group. With their participation in this special "founders" group, the participating practices have extended and deepened their commitment to complete transformation.

Touchstone Group practices are assigned to one of three teams and each team has their own designated practice facilitator. In addition to participating in monthly conference calls the practices also participate in performing practice measurement processes facilitated by TransforMED.

This continued collaboration provides:

• An opportunity to build upon the NDP's real-world picture of what the Patient-Centered Medical Home model of care looks like in a variety of settings and locations
• Continued peer-to-peer learnings
• Additional opportunities for practice metrics – both in terms of quality of care provided and the practice finances
• Continued opportunities to demonstrate for third party payers and employers the high quality care provided by family physicians
