Practice Enhancement Facilitators
In order to help create a truly "relationship centered" practice model, TransforMED's Practice Enhancement Facilitators work directly with the family medicine practice team. The facilitators are highly skilled in business, research and health care administration and each one is personally committed to providing a strong and effective communication link between the practice team and TransforMED.
