TransforMED Mission and Values

Mission

The mission of TransforMED is to lead and empower medical practices in implementing the new model of patient-centered care — thereby improving health care for their patients, as well as the success of their practices.

Objectives

• Develop high-performance primary care practices through a transformative process of practice redesign focused on patient care and practice team satisfaction
• Generate transportable new knowledge about the practice transformation process
• Generate means to allow for the continued financial viability of the organization

Values

TransforMED conducts its activities with integrity. TransforMED is a credible authority that speaks honestly, reports truthfully and conducts its business ethically. TransforMED takes safety, security and privacy very seriously and continuously works to keep all data and tools secure.

TransforMED's work is founded on research and science. TransforMED supports education, and respects experience and diversity.

TransforMED is an active member of the family medicine and primary care communities and recognizes and supports the unique value of the practice of family medicine and primary care to patients and the health care system. We share the special idealism and altruism of family and primary care physicians and their strong commitment to their specialty, communities and patients through continuing patient relationships and independent decision making.

Operational Transparency

The TransforMED team believes the success of implementing the TransforMED Medical Home Model and genuinely helping to improve family medicine depends in part on the team’s commitment to transparency. All details of the project – financial, research results and more – will be fully disclosed and all transactions will be transparent to the greater health care community.
