22 TransforMED PCMH Practices Receive NCQA Recognition

Transformation to a patient-centered medical home (PCMH) is a challenging task in and of itself, not to mention that payers, often require participating practices to achieve NCQA recognition in order to qualify for enhanced reimbursement within a pilot project. 

As an expert in medical home transformation, TransforMED is tasked by the practices we work with to help them not only with their medical home initiatives, but also to assist them in the NCQA PPC-PCMH™ recognition process.

This was the case with 22 of the practices TransforMED worked with in 2009 and we’re proud to announce that we helped these practices reach and often exceed their NCQA recognition goals — in less than a year.

How TransforMED helped these practices achieve NCQA recognition – in the words of the Practice Enhancement Facilitators:

"We made ourselves available throughout the first 9 months of 2009 for any questions or to review their submission data.  We provided conference calls for reviewing all the NCQA elements and then had individual teaching and coaching with practice managers and doctors."
