TransforMED Refines its Model of Care10/05/2009
In much the same way that primary care treats the whole person, the TransforMED Patient-Centered Model of Care recognizes that all the elements of a successful Medical Home practice are interrelated, thus the TransforMED approach "treats the whole practice." Our comprehensive model unites the enduring relationship-centered values of primary practices with new technologies and approaches to enable practices to better serve the needs of both patients and practices. Reacting to learnings from multiple Medical Home pilots and comments coming from health reform policymakers, the TransforMED model has recently been refined. The Practice Services module is now titled "Practice-Based Services" and the Continuity of Care Services module has been changed to Care Coordination. Within the Care Coordination module, TransforMED incorporated emergency room visits to its list of collaborative relationships necessary for a Patient-Centered Medical Home transformation. Dr. Terry McGeeney, President and CEO of TransforMED offers his perspective on the changes to the model: Practice-Based Services "As we conduct Medical Home pilots across the county and continue to evolve and change here at TransforMED, so will our Patient-Centered Model of Care. We changed "Practice Services" to "Practice-Based Services" in order to clarify that the elements described in this module are services delivered within the primary care practice." Care Coordination "Care Coordination is one of the core responsibilities in primary care, thus the new module name unequivocally situates that responsibility within the Medical Home. Within the Medical Home, practices want to make sure there's a timely and well thought out relationship for the exchange of information both to and from other entities that work with the primary care provider."
