TransforMED and MGMA Join Forces to Educate Primary Care Physician Leaders on Components of the Patient Centered Medical Home

TransforMED, a wholly-owned subsidiary of the American Academy of Family Physicians (AAFP) and the Medical Group Management Association (MGMA), an association committed to continually improving the performance of medical group practice professionals, will host their first National Conference together on the Patient Centered Medical Home (PCMH) at the Fort Worth Hilton in Fort Worth, Texas from November 12-14, 2009.

The conference, jointly led by prominent PCMH experts from TransforMED and MGMA, will help educate physicians, administrators and practice leaders on the essential components of PCMH and the reasons why PCMH is important to primary care practices.

"This is a conference of national importance with the leader in medical practice management and the leader in patient centered medical home and practice transformation partnering to provide a one of a kind conference," said Terry McGeeney, CEO and President of TransforMED. "It's also a valuable step for physician leaders to gain the critical knowledge they need to transform to a PCMH and become a high-performing primary care practice," said McGeeney.

Primary care practice leaders will also benefit from hearing first hand from others' experiences as they begin to examine the barriers and challenges associated with implementing the patient-centered medical home.

Other areas of interest:

  • How to relate the PCMH to the emerging changes in the health care system in your practice
  • Ways to identify patient engagement strategies to optimize self management abilities
  • How to maximize your financial viability by learning how to improve your practice's financial performance
  • Opportunities to improve the operational efficiencies of your team
  • How to manage the change process and sustain the transformation

Physicians, Administrators, and Primary Care Practice leaders who are interested in further education on areas such as access to care, practice based team care, quality and safety, care management, and health information technology should visit www.mgma.com/pcmh for complete details including speakers and registration information.
