TransforMED Partners With AAFP Chapters

April 27, 2009 – Leawood, KS - TransforMED, a wholly-owned subsidiary of the American Academy of Family Physicians (AAFP) today announced a partnership with 16 AAFP Chapters in an effort to form a stronger alliance with these state-based, family medicine-focused organizations. TransforMED is the leader in support and consultation services for primary care physicians and health systems that want to transform their medical practices into patient-centered medical homes. The AAFP represents more than 94,000 physicians and medical students nationwide and is the only medical society devoted solely to primary care.

The TransforMED AAFP Chapter Partners program will support practice change initiatives at the state level and help AAFP Chapters communicate the benefits of practice change initiatives (such as patient-centered medical home) to primary care practices in their state. TransforMED will provide  Chapters with a variety of education and practice improvement tools and resources, including TransforMED's Medical Home Implementation Quotient (MHIQ) an online self-assessment tool, Chapter special events, and discounts on designated TransforMED's products and services.

The partnership program offers an engaged relationship between TransforMED, a leader in medical practice transformation including special events and opportunities, speakers, tools and resources to AAFP Chapters who join the program.
