TransforMED’s MHIQ now offers NCQA conversion tool

TransforMED's Medical Home IQ — a new, Web-based self-assessment tool for primary care practices seeking to become Medical Homes — now offers users who complete all 8 modules the option to convert their Medical Home IQ score into an estimate of how they might score on the NCQA PPC-PCMH™ — a national recognition program based on the patient centered medical home.

The TransforMED Medical Home IQ, also known as the MHIQ, can be accessed at http://www.transforMED.com/MHIQ.

The enhanced MHIQ compares the answers to MHIQ questions with the related PPC-PCMH™ Standards, with an emphasis on the NCQA's "must-pass" elements. The conversion is a partial but important indicator of how the practice might score on the NCQA recognition tool.  

MHIQ also provides tips and resources to develop practice processes and improve the practice's performance before taking the NCQA PPC-PCMH™.

The MHIQ helps primary care and family medicine practices find out where they stand on the journey to becoming a Medical Home by measuring the practice against the eight components of the TransforMED Medical Home Model: Practice Management, Information Systems, Quality and Safety, Access to Care and Information, Team-Based Care, Continuity of Care Services and Point of Care Services.
