Job Title: Practice Enhancement Facilitator

Description: TransforMED, a growing medical practice transformation company located in the Kansas City area is seeking qualified candidates for the position of Practice Enhancement Facilitator.

Facilitators are the lead consultants who work directly with primary care practices, payer organizations and health systems to redesign the delivery of primary care to patient centered medical homes. Client engagements are focused on implementing and measuring outcomes in efficiency, financial viability, quality improvement, care coordination, health information technology, patient satisfaction and team based performance in primary care settings including family medicine, internal medicine and pediatrics.

Requirements: We are currently seeking qualified candidates with expertise in health information technology, quality management, business/financial management of medical practices. Prior health care consulting experience is a plus.

Ideal candidates will possess the following qualifications:

• Minimum of 5 years experience in health care, practice management, quality management and/or clinical care
• Masters degree in health care, business or related field preferred
• Nurse practitioner, physician assistant, practice manager, lean six sigma experience a plus
• Ability to work in a fast paced virtual high performing team environment
• Excellent organizational, time management and project management skills
• Ability to manage client engagements including contract deliverables and budgets
• Excellent public speaking/education skills
• Demonstrated written and verbal communication skills including public speaking and web based education
