Carlos R. Jaén, MD, PhDPrincipal InvestigatorCarlos R. Jaén, MD, PhD, is the overall Principal Investigator. He is a family physician and health services researcher who is co-Director of the Center for Research in Family Medicine and Primary Care. His particular area of interest is health behavioral counseling in primary care practices. Over the last 15 years he has collaborated as a co-investigator or consultant with Drs. Stange and Crabtree in multiple NIH funded grants studying primary care practices: DOPC, STEP-UP Trial, Prevention and Competing Demands, EPOCHS practice improvement and ULTRA. He is Professor and Chairman of Family and Community at the University of Texas Health Science Center at San Antonio. Throughout the three year evaluation of the NDP, he will spend approximately 20% of his time participating in the ongoing evaluation of the TransforMED National Demonstration Project (NDP.) Dr. Jaén will ensure the team meets the project objectives and will also work with other members of the NDP Evaluation Team to discuss and code ongoing qualitative data. Dr. Jaén will work with members of the TransforMED NDP facilitation team to develop a process for managing data, analyzing and summarizing the NDP, and disseminating the findings. He will supervise the Project Director, based in San Antonio. He will participate in weekly web-based conference calls with Drs. Crabtree and Nutting as well as the TransforMED NDP facilitators and qualitative analyst. He will also participate in the co-investigator retreats and learning sessions and will coordinate the administrative aspects of the NPD.
