Terri Kuhn, BSN, RN, CCRP

Practice Enhancement Facilitator

In her role as Practice Enhancement Facilitator, Terri uses data and observations gleened from TransforMED's growing list of projects and pilots to engage clients to enhance care delivery through the TransforMED Patient-Centered Model of Care.

She is one of TransforMED's expert nurse clinicians focusing on performance improvement, technology integration, regulatory compliance, team building, and workflow efficiency.

Terri earned her nursing degree from Graceland University and brings more than 20 years of nursing and leadership experience in directing innovative teams to build and sustain best practices.

She has a certification in clinical research and has had the opportunity to work in academia, private practice and hospital based healthcare systems.
