Program Director - Collaborative PCMN CMMI Project

Noelle King-WhitlowNoelle King-Whitlow serves as Program Director of the Health Care Innovation Award (HCIA) from the Centers of Medicare & Medicaid Innovation at TransforMED. The HCIA was granted to TranforMED to demonstrate the Patient Centered Medical Neighborhood (PCMN) model of care to expand integration and the coordination of care in the medical home model. Noelle is responsible for program implementation and achievement of program goals.

Noelle joined the TransforMED team immediately following graduation from the University of Kansas where she received a Masters of Science in Nursing and Masters of Health Care Service Administration.  She also received her bachelor’s degree in Nursing from the University of Kansas and currently holds a registered nurse license. She has experience in assisting primary care practices, health care systems, and Federally Qualified Health Centers (FQHCs) with medical home transformation and National Committee for Quality Assurance (NCQA) medical home recognition process.  Noelle is a leader in healthcare quality improvement & is a Certified Professional in Healthcare Quality (CPHQ).

Earlier experiences in Noelle's career include developing and implementing a Skilled Nursing Unit and an Alzheimer's Unit as a Director of Nursing in a long term care facility, a practice manager at a primary care practice, and an adjunct nursing instructor.
