Practice Enhancement Facilitator

Kathy Steen, BSN, RN

In her role with TransforMED as a Practice Enhancement Facilitator, Kathy Steen assists family medicine practices with implementing the TransforMED Model of Care. She is one of six facilitators implementing this model with practices around the country, utilizing data and observations gleaned from TransforMED's National Demonstration Project (NDP). TransforMED initiated the NDP to better understand the process of change in primary care practices moving toward the goal of becoming Medical Homes. TransforMED facilitators have developed expertise in working with practices in many areas of focus such as change management, practice finance, team-building, leadership development, practice efficiencies, developing and understanding practice metrics, and more.

Kathy's career has been a progression of nursing and healthcare management positions which have allowed her to experience the delivery of healthcare in a variety of settings. Early in her career, experiences on Vanderbilt University Medical Center's Cardiac Step-down unit convinced Kathy that her patients needed more than outstanding medical management— in order to live well with chronic disease they needed whole person care that could not be effectively delivered solely in an inpatient setting. As a result, Kathy's focus began to shift to chronic disease management and then to prevention. Leaning on her proven successes in these arenas, Kathy joined a team dedicated to building an integrated family practice clinic and fitness center founded on the Future of Family Medicine report. Having lived 'in the trenches' of new business launches, Kathy is well equipped to assist others as they navigate the challenges of crafting their practices from the ground up. Kathy also brings a unique 'insider' perspective to TransforMED— having led one of TransforMED's 18 National Demonstration Project (NDP) practices, Kathy has experienced first hand the benefits gained as a result of 'transforming' a practice. In ways that few can, she fully appreciates the courage and commitment required to bring about strategic change.

Kathy is a graduate of Middle Tennessee State University with a BS in Nursing.
