Practice Enhancement Facilitator

Jettie Eddleman, BSN, RN

In her role with TransforMED as Practice Enhancement Facilitator, Jettie engages family medicine practices and TransforMED clients to enhance care delivery through the TransforMED Patient-Centered Model of Care.

She is one of TransforMED’s expert nurse clinicians focusing on performance improvement, quality systems metrics as well as patient safety and engagement initiatives within practices.
 
Jettie brings more than 20 years of health care administration and has experience in directing innovative teams to build and sustain best practices.
 
Her early contributions to health care on the local and national stage were based out of the Dallas/Fort Worth, Texas area, and most recently, in Austin, Texas. After studies at University of Texas at Austin, and earning a Bachelor of Science in Nursing from the University of Texas at Arlington, Jettie studied at Texas Christian University in Fort Worth.
