Elaine M. Skoch, RN, MN, CNAA, EMBA

Practice Enhancement Facilitator

As a Practice Enhancement Facilitator, Elaine works with the other TransforMED practice enhancement facilitators to develop performance improvement processes within the TransforMED practices. Practice indicators to evaluate the efficiency and effectiveness of practice operations, the quality of care and the outcomes of care as well as the risk associated in the delivery of care are being developed and implemented. Elaine continues to pursue the development of a "dashboard" of indicators for use by practices to assist in data-based decision making, best practices, and good quality information to assist practitioners and their staff to optimize the way they practice family medicine. The educational aspect of Elaine's position includes working with a variety of professionals in a number of different venues to disseminate, implement and integrate the principles, processes and structures of the Patient-Centered Medical Home.

From the beginning of her professional career, Elaine has been committed to caring for the "whole person." Her clinical nursing experience has been in many arenas; from ICU and burn trauma to the provision of mental health and substance abuse services for indigent and homeless clients in the inner city. Elaine has also served in administrative and leadership positions, in public and private healthcare settings. She has worked to improve services for populations of clients who are chronically mentally ill and has been involved in performing major systems analysis and reorganization, organizational development, long range planning, marketing, change management, budgetary oversight, and program evaluation through continuous quality improvement initiatives.

Many of the elements of the TransforMED model reaffirm the ideals Elaine values: empowering people to participate in their care and form a partnership with their health care professionals; the commitment to quality and efficiency in the delivery of care with emphasis on prevention and wellness; and, that access to care should be readily available to everyone.

Elaine has an Executive MBA from Benedictine College and a Master of Nursing from the University of Washington. She is Board Certified in nursing administration, at the advanced level, through the American Nursing Association Credentialing Center.
