Practice Enhancement Facilitator

Diane Cardwell, MPA, ARNP, PA-C

In her role with TransforMED as a Practice Enhancement Facilitator, Diane assists family practices with implementing the TransforMED Model of Care. She is one of six facilitators implementing this model with practices around the country, utilizing data and observations gleaned from TransforMED's National Demonstration Project. The Project was completed by TransforMED facilitators to create specific changes in family practices and to assess the effectiveness of the methods used for doing so.

Diane brings to her position and the practices she works with 15 years of primary care clinical experience. In addition Diane has worked in an administrative role in a multi-specialty clinic working with physicians, NP/PA providers and staff. Her experiences include working with solo provider practices in rural settings and primary care departments in a large multi-specialty group. She brings firsthand experience with efficient NP/PA/Physician team care, policy development, open access scheduling and many coordination of care components.  

Diane received her MPA from Drake University, ARNP from University of North Dakota and her BSN from Fort Hays State University.
