Chris Rodrigues

Project Manager

In his role with TransforMED as a Project Manager, Chris serves as a team member working directly with Practice Enhancement Facilitators as they support the family medicine practice teams in their effort to transform to the TransforMED Patient-Centered Model.  

In addition to managing transformation activities and ensuring effective implementation, Chris also provides on-site practice coaching and facilitation services.

Chris comes to TransforMED with 10 years of behavioral experience, including case management and program manager of rehabilitation services.

In addition to his behavioral health background, Chris was the Information Technology and Project Director for the Rhode Island Health Center Association, the state’s primary care association (PCA) for Federally Qualified Health Centers (FQHC), Federally Qualified Look Alikes, and community health centers.  In his role as IT and Project Director, Chris managed all information systems and data analysis, company web site, and infrastructure.

Chris holds a Bachelor’s in Psychology with a minor in Sociology from Wheaton College in Norton, MA and a project management certificate from Bryant University in Smithfield,  RI.
