Project Manager - Collaborative PCMN CMMI project

Cheryl Scott, MSMCheryl Scott serves as TransforMED's Project Manager of the Health Care Innovation Award (HCIA) from the Centers of Medicare & Medicaid Innovation. The HCIA was granted to TransforMED to demonstrate the Patient-Centered Medical Neighborhood (PCMN) model of care to expand integration and the coordination of care in the medical home model.

Cheryl is responsible for partnering with key stakeholders to design, develop, and implement all aspects of transformation towards high performing Patient-Centered Medical Homes and the Medical Neighborhood.

Cheryl’s background includes system development and project management in the telecom industry. Cheryl is an adjunct faculty instructor at Friends University in the business degree program and active in the volunteer community.

Cheryl earned a B.A in Human Resources and Management from MidAmerica Nazarene University and a Master of Science degree in Management from Friends University. Cheryl also earned a Master Certification in Project Management from George Washington University.
