TransforMED MHIQ Scoring

MHIQ Performance Levels Key Needs significant improvement : The practice is taking the first steps toward becoming a medical home. Needs improvement: The practice continues to improve and successfully completes some significant transformations. Good: The practice is well on its way to becoming a TransforMED practice – a true medical home. Advanced: With only minimal improvement the practice can be recognized as a Patient Centered Medical Home.

Module Scoring
The MHIQ tool identifies the status of the practice as a medical home through a series of simple questions across 8 modules that are based on the TransforMED Medical Home Model.

Based on the responses to questions, the MHIQ generates a report containing an indicative analysis on the status of the practice for the designated module. The module assessment also groups the practice's score into one of four areas of recommendations on how to proceed.

When the practice completes all 8 modules, the online application generates an overall medical home assessment score with recommendations grouped into the same four categories described above.

MHIQ-to-NCQA PPC-PCMH™ Standards Comparison
In addition to receiving a score on individual modules and the completed assessment, users may also choose to compare their MHIQ answers to the NCQA PPC-PCMH™ must pass elements.  The comparison does not constitute NCQA Recognition or a score for the PPC-PCMH™.

The NCQA PPC-PCMH™ includes 30 elements, of which 10 are "must pass," meaning you must pass them all to achieve Level 2 and Level 3 recognition regardless of your total PPC-PCMH™ score. The MHIQ compares your answers to PPC-PCMH™-related questions on the MHIQ with emphasis on the "must-pass" elements, and therefore provides a partial but important indicator of how the practice might score on the NCQA recognition tool.  MHIQ also provides tips and resources to develop practice processes and improve the practice score before taking the NCQA PPC-PCMH™.
