CareFirst BlueCross BlueShield

In January 2009, TransforMED began working with CareFirst BlueCross BlueShield (CareFirst) and 11 primary care practices in the company’s Mid-Atlantic service area in a pilot implementation of the patient centered medical home model of care (PCMH).

These 11 practices represent internal medicine, family practice, and pediatrics. TransforMED will be working with these practices for two years to allow sufficient time to develop the necessary human and technological infrastructure necessary for such a program.

This project will explore what difference this model makes for improving outcomes and for reimbursing primary care at a higher level. All the parties involved in this pilot understand that to be competitive in the 21st century requires a paradigm shift from thinking about the patient within the frame of an acute care episode, to thinking about caring for the entire population of patients. To do this requires a new way to think about team care in addition to key technological support.

This Spring we had our first all practice meeting where all the parties involved in this pilot were able to network with one another while learning more about the PCMH with a specific focus on care team innovations and improving access.
