TransforMED, The Innovator

TranforMED has been highly successful and receives broad national recognition for being the “fixer” or “transformer” of primary care practices.  Many don’t realize that TransforMED has an equally important impact on both payers and healthcare systems across the country.   The insurers in many of our pilots have changed their systems with our guidance to keep primary care providers better informed and engaged, expedited or in some cases eliminated burdensome prior authorization requirements, explored new ways to expedite payments to practices, redesigned member benefit plans to promote transformed primary care practices and of course tested or even implemented new compensation models for primary care practices. 

Hospital systems likewise have explored new compensation plans for employed primary care physicians, implemented new systems to support “affiliated” practices, promoted the value of primary care in their systems, insured that primary care remains the foundation of their ACO initiatives and leveraged technology to coordinate care in the “medical neighborhood” environment.    Furthermore, TransforMED has become quite successful with a variety of small practice solutions to support small, independent practices in a number of environments across the country.  Add to that the fact that TransforMED is recognized as the national expert to support practices in gaining NCQA recognition and you have an unmatched track record of success.

The end result is that TransforMED is much more than the “fixer” or “transformer” of primary care practices.  TransforMED has been referred to as the “Gold Standard of Medical Home”.  I would suggest that TransforMED is also the gold standard and leader in primary care and health system innovation.  One only has to reflect back to the beginning of TransforMED when it took the Future of Family Medicine report’s “New Model of Care” and created a model document that evolved and later became this “gold standard” of Patient-Centered Medical Home.  TransforMED had the foresight to include practice management as a component of its PCMH model understanding that creating time and financial efficiencies for the practice is critical to success.  TransforMED also leveraged and expanded a blended facilitation and collaborative meeting model coupled with multiple layers of technology innovation to support primary care practices across the country.  This was done with a staff of highly skilled individuals who were not entrenched in the traditions of healthcare, but educators, practice management and business experts, leadership and change management experts and of course individuals with very strong clinical backgrounds. TransforMED staff members are not the typical “consultants”, but an innovative, broad-based way to support primary care practices.  TransforMED has advised on and tested a number of new payment methodologies in its pilots and projects.  TransforMED has developed multiple webinar series, workbooks, an on-line learning community via the Delta-Exchange, a multitude of practice analysis tools, and on and on.

More than just the “transformer” of primary care and the healthcare system, TransforMED is truly the ultimate innovator.  TransforMED has always had the goal to lead, not follow.  This aligns perfectly with its mission of the “transformation of healthcare delivery”.  As the world is quickly evolving, practices, health systems, payers and the government look to TransforMED as the innovator to create visions, roadmaps and a clear plan to success.  TransforMED is in the unique position to innovate in ways unique to a variety of healthcare stakeholders in varied environments.
