The PCMH Code: It isn't a secret, but it is hard work

A common question asked of TransforMED is "What is the key to successful transformation to a patient-centered model of care?" It's a great question and one whose answer requires in-depth understanding of the data coupled with a keen recognition of the complexities of practice dynamics.

I think the key is for practices to engage in meaningful evaluation and reflection, followed by meticulous planning and calculated execution. Below are four critical steps practices can take to succeed in this process.

  1. The first step in the journey to a patient-centered practice is to fully understand the patient-centered model of care and where your practice currently resides within that model. The MHIQ (Medical Home Implementation Quotient) is where a practice should start. It is a free tool on the TransforMED website that provides a thorough description of the model while allowing a self-assessment of where the practice is currently.

  2. The second step is to fully appreciate the critical success factors for a meaningful practice transformation. They are: change management, leadership, communication and the ability to function as a team within the practice. These issues must be carefully reflected upon and meaningfully addressed prior to substantial change in a practice — or havoc will ensue.

  3. The third step is for the practice to sit down as a team and use the TransforMED model to develop a project list and timeline for the transformation to a complete medical home. I have seen data that suggest there is limited value to a partial medical home and that should never be the initial goal. All of the components of the patient-centered model are interdependent and improvement in outcomes and efficiency is not typically incremental.

  4. The fourth step is implementation and transformation based on the project list. It is recommended that the practice work toward early successes and celebrate the successes. It generally works best to get some early wins with something like a practice website or extended hours.

While some practices stumble addressing the critical success factors effectively and openly, many more are challenged at the transformation phase. Meaningful change in a practice that is busy and overworked is difficult.
